Chronic osteomyelitis

Patient History : Road traffic accident 10 year back, operated - Rod with screw fixation - Now discharge from medial aspect of distal leg.
Gender:Male
Age:76 Y

-          Tibia shows cortical thickening with loss of normal marrow signal intensity in the mid and distal shaft region. Marrow edema with fluid noted on T2W / STIR images. 
-          Few tiny bony fragments noted in the medullary cavity – suggestive of dead bone (sequestrum). 
-          Defect (Cloaca) measuring 6mm noted in the medial aspect of mid shaft of tibia through which the medullary cavity is seen communicating with the skin subcutaneous region through a sinus tract of length 1.4 cm. 
-          Subtle area of restricted diffusion noted in the medullary cavity.
