Large lobulated well enhancing extra-axial mass lesion is noted in the right petroclival region with following details:
- It measures 72mm (CC) x 49mm (TR) x 54mm (AP) dimensions.
- Superiorly, the lesion is extending upto the level of 3rd ventricle with significant mass effect on the thalamus.
- Anteriorly, it is encasing the right cavernous sinus with intrasellar and right parasellar extensions. Laterally, it is causing compression over the right medial temporal lobe.
- Medially, it is encasing the basilar artery. It is causing significant compression over the pons and mid brain. The right lateral ventricle is compressed with midline shift towards the left by 6mm.
- Inferiorly, it is extending into the right CP angle cistern and premedullary cistern and partially abutting vertebral arteries. It is focally extending into right internal auditory canal. There is erosions of the adjoining petroclival bones. The right middle cerebellar peduncle and right cerebellum are partially compressed. The 4th ventricle is displaced towards the left side of midline.
- Perilesional white matter edema noted in the right temporal lobe, right ganglio-thalamic region, hypothalamic region, right periventricular region, brainstem and cerebellum.
