MR defecogram Rectocele

Patient History : Difficulty passing stools
Gender:Male
Age:38 Y

 
MRI DEFECOGRAM
 
Protocol:
Dynamic imaging of pelvic floor was done in sagittal plane using T2W sequences during rest, squeezing, straining and defecation. 
 
OBSERVATIONS:
 
Pelvic floor imaging:
 
At rest: The anorectal junction is abnormally low ( 44mm below the pubococcygeal line). Anal sphincter is closed without leakage of endorectal material. Ano-rectal angle measures 1090. 
The positions of the bladder base normal and noted above the pubococcygeal line. 
 
During squeezing: The ano-rectal angle closes to 95 degree on squeezing. The anorectal junction remains abnormally low (49mm). 
 
During straining: Further medium descent of the rectum noted. No obvious descent of the urinary bladder.
 
During defecation: There is further  descent of anorectal junction.  There is also mild descent of urinary bladder neck  below pubococcygeal line. No significant rectocele.
 
 
IMPRESSION:  MR Defecogram Study reveals:
 Moderate prolapse of posterior compartment (Rectal) and mild prolapse of the anterior compartment (urinary bladder).
